Mr. Kelton is a 45 yr old man that lives in the district. He is alert and oriented x3, verbalized skin was intact without cut or bruise. Client cannot remember last medical appointment visit. Client denied suicidal ideation or homicide. He has history of post traumatic stress disorder and schizoaffective bipolar type. Presently, client verbalized he was homeless and performed activities of daily living in a shelter or friends house. Client verbalized to be stable until his mother died.
